How to make a supported living referral

Send us what you have. We will tell you quickly whether we can help, what else we need, and what happens next — without making you chase.

Referrals are welcome from local authorities, ICBs, case managers, families and self-funders.

Who can refer

Anyone can start a conversation with us. Formal referrals most often come from local authority social work and brokerage teams, ICBs and Continuing Healthcare, hospital discharge and community mental health teams, case managers and solicitors — but families and self-funders can approach us directly.

You do not need a complete pack to get a useful answer. An early phone call with the broad picture is often more valuable than a full referral form three weeks later, because it tells you quickly whether it is worth assembling the pack at all.

What to send us

  • A summary of the person and their support needs
  • Current or most recent needs assessment
  • Risk assessment and any positive behaviour support plan
  • Relevant health information, including medication
  • Communication passport or equivalent, if there is one
  • Details of the funding route and who holds responsibility
  • The property, or the housing route being pursued
  • Named contacts across health, social care and housing

If some of this does not exist yet, send what you have. We will tell you what is genuinely needed to make a decision.

What happens next

The referral pathway

    You get in touch

    By phone, email or the referral form. Whichever is quickest for you.

    Initial review

    We review the information against what we can safely deliver and come back to you with an early view.

  3. 3

    Assessment

    If it looks viable, we arrange an assessment with the person, in the setting they are currently in.

  4. 4

    Proposal

    We set out what we would provide, the staffing model, and the costs — clearly enough for a panel to make a decision on it.

  5. 5

    Funding agreement

    Confirmation of who is funding what. Nothing starts before this is settled.

  6. 6

    Transition planning

    A move plan with a named lead and a realistic date. See our transition process.

  7. 7

    Support begins

    With closer oversight in the early weeks and review points already booked.

What you can expect from us

A clear answer, either way

If we cannot support someone we will tell you promptly and explain why, so you can go elsewhere without losing time. Providers who go quiet on referrals they cannot take cost commissioners weeks.
